New Patient Forms

Welcome New Patients!

Thank you for choosing our practice for your skin care needs. We offer a wide range of medical, surgical and cosmetic dermatological services, and we will be happy to address concerns in any of these areas.

The first step is to call our office at 205.877.9773 to set up your appointment. Or, if you would like for our scheduler to contact you, click here.

When you call to make your appointment, we will obtain your email address and will email you a security code. The security code will allow you to complete all of your new patient forms online, so that we will have your information in our system immediately without you having to print anything or fill out forms in our office! Please bring any previous medical records that

may be beneficial or pertinent to your visit. You may also request that your records be faxed to us at 205.877.9775 prior to your visit.

Our goal is to schedule appointments in an efficient and timely manner. Please call us at least 48 hours in advance should you need to cancel or reschedule your appointment. We send appointment reminders via text 2 days prior to your scheduled appointment time. If you miss your appointment without advance notification, your account is subject to a fee.
